Home Care is more than just taking care of the physical needs of the client. There are emotional needs that also come into play. TCAPC provides a clinical management model that incorporates Social Workers and Nurses to help coordinate all the needs of the client.Families and clients are taught the cares as appropriate so they can feel more confident in the care delivery of the client in the event that staffs is not available or family wants family time.
Skilled Nursing Visits are ordered by the consumer’s physician.Skilled Nurses observe, assess, and evaluate consumer’s physical and mental state and provide health care training to consumer and family care givers when appropriate.Each patient is different; however, a typical Skilled Nurse Visit is 30 min-4 hours per visit depending on physician orders.
These services are intended to help patients develop and enhance their current physical, occupational, and speech condition. Therapy Services can be done in an inpatient setting, outpatient setting, or provided in the comfort of someone’s home by a qualified home care agency.All therapy services must be ordered by a physician.
HHAs are individuals that are trained by qualified home care agencies to help consumers with their Activities of Daily Living; including but not limited to transferring, getting out of bed, eating, toileting, bathing, grooming, dressing, light housekeeping, etc..
Extended Hour Nursing Services (also known as Private Duty Nursing Services) are deemed medically necessary by a physician with proper orders and authorization to qualifying agencies.
Qualifying agencies assess consumer’s cares for appropriate nursing hours and develop a plan of care which is then authorized by the consumer’s physician and appropriate payer source.
Registered dietitians at Tcapc know how vital a healthy, nutritious diet is to health and well-being. Whenever needed we provide personally tailored and monitored meal planning and special diets.When participants visit Tcapc, they receive meals. Meals also may be home-delivered.
